This award was formally presented at the Population Health Pioneer Awards banquet dinner during the third annual Caravan Health Accountable Care Symposium on December 5-6, 2018 in Phoenix, AZ. The much-anticipated Symposium will feature thought leaders, industry experts, panel discussions, and breakout sessions that address the most pressing concerns of health system leaders today.
